Assignment information can be obtained on applications processed by the Plan from the Reprint Application/ID card services option. You must log in to the online processing system in order to access this option.

ID cards generated in conjunction with a PASS application which are reprinted after an assignment has been issued, will include the name of the assigned carrier, the APN number and the effective date of coverage as it appears on your assignment notice. The Department of Motor Vehicles (DMV) has approved these enhanced reprint ID cards as acceptable for proof of coverage for resolving a DMV suspension for lapse of coverage notice.
